If you have been notified that your health insurance policy will be or has been cancelled, and you believe that health plan options available through the Marketplace are unaffordable, you may be eligible for a hardship exemption to enroll in catastrophic coverage available in your area. Answers to some frequently asked questions about the hardship exemption for consumers with cancelled policies are available on the Resources page.
• If you enrolled in coverage effective January 1, 2014, you should have received your invoice. If you make payment within 10 days of receiving your invoice, your coverage will be effective on 1/1/14. • If you received your premium invoice and made payment, but have not yet received an ID card, your coverage is in effect. You will receive ID cards over the next several days. You can access a temporary ID card via the Empire website within 24 hours after Empire receives your payment. • If you were previously enrolled in another plan through Empire and received an invoice that was not for the plan you selected through the Marketplace, you should have received a new invoice that includes any tax credits you qualified for. • A special notice has been sent to Healthy NY members who were automatically enrolled into an Off Marketplace plan explaining their options and clarifying any premium invoices which have been received. • If you have additional questions about your coverage with Empire, please contact Empire Customer Service at 1-855-748-1806. Empire has recently added additional staff and extended call center hours.

There are two ways to find out if your health care providers (like doctors) and facilities (like hospitals) participate in Marketplace plans. Anyone can search the Health Plan Provider Networks resource posted on this website. Use the links in that attachment to access the provider networks of each health plan offered on NY State of Health. These links will bring you to the websites of NY State of Health plans. From there, you can search for your providers. Or, as part of the application process, you can search for plans that your health care providers and facilities participate in. Plan provider networks can change from time to time. Verify the participation of your provider when enrolling in a health plan. If you need more information about a health plan's benefit or have specific questions about a plan's network, you can contact the health plan at the phone number provided.
